BackgroundIt is increasingly commonplace to involve relevant professionals and people with lived experience within healthcare research. Although valuable case studies regarding such involvement exist, there is currently a paucity of case studies highlighting the professional and personal impacts of uniting a diverse group of professionals and people with lived experience within the development of new, theory-based interventions. The aim of this paper is to provide insight into the impacts of involving a diverse range of individuals, unified within a single steering group, within the development of a new, theory-based, paediatric speech and language therapy intervention (“Supporting Words and Sounds” - SWanS). By describing the involvement process in detail and providing our personal insights, we hope our recommendations will be of use to future healthcare researchers.Main bodyOur project steering group consists of two people with lived experience (an adult with Developmental Language Disorder-DLD, a parent of a child with DLD), three specialist NHS speech and language therapists (including one university lecturer with equality and diversity expertise), and two individuals working in the education sector (a specialist teacher and a bilingual educational support worker). Group members have been involved across the 4 phase intervention development process. Tools such as the PiiAF (Public involvement impact Assessment Framework) have guided our personal and professional reflections on our individual experiences of being in a diverse steering group responsible for developing a new and complex theory-based intervention.ConclusionWe found that having a diverse range of people unified in a singular intervention development steering group had unexpected benefits. Learning from each other has enriched professional practice and developed individuals’ confidence in terms of playing an active role in research. Our structured reflection has implications for future intervention development research, by highlighting that the provision of a safe, supportive space and nurturing of shared values is key when involving a diverse range of parties. Such contexts promote sustained involvement and therefore have longer term implications for increasing the relevance of the research for those it is aiming to help.

The provision of critical ecosystem services like carbon sequestration by peatlands has been degraded around the globe. Peatland restoration represents an opportunity to tackle the twin global emergencies of climate breakdown and biodiversity decline. Nonetheless, restoration success relies on a sound understanding of recovery trajectories associated with different restoration techniques. Focusing on temperate/boreal Sphagnum‐dominated peatlands, we used a quantitative meta‐analysis of 28 studies representing 275 sites in 11 countries to test for effects of peatland status (intact, restored, and degraded), varying restoration interventions and time since restoration on vegetation as a key indicator of peatland condition and functioning. Enhanced restoration (such as active revegetation) resulted in recovery to predisturbance levels within 30–35 years for Sphagnum mosses, and 20–25 years for many other peatland specialist species, and was the only restoration approach where positive outcomes were seen across all vegetation response variables. The use of standard restoration techniques, such as rewetting, was projected to result in cover of Sphagnum mosses and peatland specialist plants reaching that of intact sites within 45–55 years post‐restoration. Passive restoration (cessation of the degrading activity with no active restoration) generally elicited limited recovery of keystone peatland vegetation (Sphagnum spp.) even after multiple decades. A lack of standardization in monitoring severely constrains the analysis of peatland restoration outcomes. Increased funding for monitoring and reporting outcomes, and improved monitoring consistency, could greatly enhance our understanding of peatland restoration ecology and improve practice.

I thank the authors of "QSEN in an Amazon World" (Special Feature, March) for sharing their perspectives on how nurses can evolve professionally in light of new changes affecting health care, and for clearly defining what is expected of us in order to become effective change agents who are able to anticipate needs and help frame the health care of tomorrow using new cognitive approaches. I found the authors' work very engaging. Fabien Wecker, MSN, MA, RN Woking, Surrey County United Kingdom

The implementation by diagnostic laboratories in England of polymerase chain reaction (PCR) to screen faecal specimens for Shiga toxin-producing Escherichia coli (STEC) has resulted in a significant increase in notifications mainly due to non-O157 strains. The purpose of this study was to develop an approach to public health risk assessment that prioritizes follow-up to cases caused by haemolytic uraemic syndrome (HUS) associated E. coli (HUSEC) strains and minimizes unnecessary actions. Epidemiological and microbiological data were prospectively collected from 1 November 2013 to 31 March 2017 and used to compare three risk assessment approaches. A history of HUS/bloody diarrhoea/age under 6 years and faecal specimens positive for stx-predicted HUSEC with a diagnostic accuracy of 84% (95% CI; 81-88%). STEC isolated by Gastrointestinal Bacteria Reference Unit (GBRU) and stx2 and eae positive predicted HUSEC with a diagnostic accuracy of 99% (95% CI; 98-100%). Risk assessment combining these two tests predicts the most efficient use of resources, predicting that 18% (97/552) of cases would be eligible for follow-up at some stage, 16% (86/552) following local stx PCR results, 1% (7/552) following GBRU results of stx2 and eae status and 0.7% (4/552) following whole-genome sequencing. Follow-up could be stopped in 78% (76/97) of these cases, 97% (74/76) following second stage risk assessment. This three-stage risk assessment approach prioritizes follow-up to HUSEC and minimizes unnecessary public health actions. We developed it into the algorithm for public health actions included in the updated PHE Guidance for management of STEC published in August 2018.

The module starts with examining the conceptual framework of commercial exploitation of children, which comprises abuse by the adult and remuneration in cash or kind to the child or a third person or persons, where the child is treated as a commercial object. It includes child labour, trafficking and sale of children and commercial sexual exploitation of children. The module then discusses the causes and effects of child labour in industries, construction work, restaurant work, domestic work, household-based work, etc., and facilitates development of awareness of child rights to prevention of their exploitation for unpaid/underpaid labour. It further discusses the causes and effects of child trafficking and sale for begging, labour, prostitution, adoption, entertainment, sale of organs and marriage followed by development of awareness of child rights to their prevention.

The module starts by examining the conceptual framework of conflict with law in childhood. The types of conflict with law in childhood are status offences, petty/property offences and violent offences, and causes are scientific construction of “normal” childhood and adolescence, testing of boundaries in adolescence, poverty, neglect and abuse, learning disabilities and mental health disorders. Based on the causes, the strategies to prevent conflict with law in childhood are identified. It then develops awareness of rights of children accused of conflict with law with reference to apprehension and trial. Finally, the module reviews the Juvenile Justice Act of India with reference to role of the police and the Juvenile Justice Board with reference to rights of children accused of conflict with law.

A GREAT deal of effort has been made to help nurses embed the principles of the Mental Capacity Act into their practice. This book is not addressed specifically to nurses, who can read a similar guide by Graham and Cowley, but will be of interest to psychiatrists.
