Gondolatok a piaci kereskedelem fejlesztésének szükségszerűségéről, lehetőségeiről és esélyeiről
Az első részben arra kerestem a választ, mi jellemzi elméleti megközelítésből a kereskedelem működését, milyen új folyamatok játszódtak le az elmúlt 15-20 évben, mi jellemzi napjaink kereskedelmi technikáit, hogyan tudna versenyben maradni a hagyományos piaci élelmiszerkereskedelem a konkurens élelmiszerláncok kereskedelmi technikáival szemben, mi az a hozzáadott érték, ami a hagyományos piacokon, vásárcsarnokokban ma még felfedezhető és megőrzésre vár. A piacokra és csarnokokra vonatkozó analízis után a piaci kereskedelem jövőbeni fejlesztési stratégiájára és egy projekt elemeinek felvázolására tettem kísérletet az előző részben. A második részben a jelenlegi állapotok feltárása és bemutatása a cél. Budapesti élelmiszerkereskedelem taglalásánál azokat az összefüggéseket és adatokat vizsgálom meg, amelyek a főváros kerületeiben működő piacok, vásárcsarnokok, valamint a különböző élelmiszerláncok üzleteinek számát, azok eloszlását mutatja be. Ezt követően a lakosság élelmiszer beszerzési döntéseit nagyban meghatározó fogyasztói árak alakulását vizsgálom néhány gyakori élelmiszernél, amelyek mindegyik vizsgált kereskedelmi területen megtalálhatók és így azok egymáshoz való viszonya befolyásolhatja (más fontos tényezők mellett) az egyes kereskedelmi egységek jövőbeli perspektíváit is. (Az első részben elméleti és gyakorlati összefüggésekkel, az élelmiszervásárlást meghatározó egyéb területekkel, tényezőkkel is foglalkoztunk.)
Read moreCBS Cards: A Game-Based Supplementary Instructional Tool for Senior High School Mathematics Teachers
Students in Senior High School often struggle with key topics in General Mathematics 11, particularly equations, inequalities, and functions. To make learning more engaging and compelling, innovative instructional materials are needed. To address these difficulties, this study explored the use of CBS Cards, an educational card game designed as a supplementary instructional material to enhance learning and engagement in Mathematics. A mixed-method research design was employed, using a survey questionnaire to gather quantitative feedback from 15 Mathematics Teachers and 15 Expert Teachers selected through purposive sampling. At the same time, qualitative data were obtained through evaluators' and students' written comments and suggestions. Findings revealed that the least mastered competencies were solving rational equations and inequalities, solving exponential equations and inequalities, and performing operations on functions. Both groups of evaluators rated CBS Cards as highly acceptable, with grand weighted means of 3.97 (teachers) and 3.98 (experts). Statistical analysis indicated no significant difference between the two groups' evaluations, suggesting consistency in their assessment of the material. Qualitative feedback highlighted the game's potential to enhance numeracy skills, promote interaction, and boost student motivation. CBS Cards present an innovative and engaging approach to addressing persistent difficulties in General Mathematics. By integrating play with learning, they can enhance participation and mastery of complex concepts. Future studies may focus on testing the effectiveness of CBS Cards in actual classroom implementation and exploring their impact on student performance through experimental studies.

Hyponatremia is one of the most common ion disorders encountered in everyday practice. It is usually caused by sodium loss or hypervolemic dilution, and side effects of drugs may also contribute. Less well known is the hyponatremia associated with hypothyroidism. The mechanism of its development is not well understood, but it is mainly explained by a decrease in myocardial pump function and secondary arginine vasopressin stimulation-induced fluid retention as wells as a decrease in renal fluid excretion. Although hyponatremia is not a typical and uncommon symptom of hypothyroidism, it may be important to be aware of the possibility of hypothyroidism underlying ion imbalance. In our case report, an elderly patient with hyponatremia was found to have pituitary insufficiency, which caused hypothyroidism and consequent hyponatremia by a central mechanism. Thyroid-stimulating hormone levels in the normal range did not initially raise suspicion of the disease, but free hormone determination revealed an underlying hypothyroidism, the central origin of which was confirmed by imaging. Háttér és célkitűzések: A családi kapcsolatok minősége kiemelt jelentőségű a gyermek fejlődése, mentális egészsége szempontjából. Angolszász nyelvterületen számos kérdőív áll rendelkezésünkre a témában. Egyikük a Családi Kapcsolatok Teszt (Family Relations Test, FRT), mely az anyához és apához fűződő kapcsolat minőségét vizsgálja a gyermek perspektívájából. Kutatásunk célja az FRT magyar nyelvű változatának elkészítése és faktoriális validitásának elemzése volt. Emellett vizsgáltuk a gyermek szüleihez fűződő kapcsolatának összefüggését az internalizáló és externalizáló tünetekkel, a proszociális viselkedéssel, valamint az életminőséggel. Módszer: Kutatásunkat nem-klinikai mintán végeztük, 10 és 18 év közötti fiatalok adatait elemeztük (N = 217) megerősítő, majd feltáró faktorelemzéssel. A papíralapú vagy online kérdőívcsomag az FRT mellett egy rövid demográfiai kérdőívet, a Gyermek Életminőség Skálát (ILK) és a Képességek és Nehézségek Kérdőívet (SDQ) tartalmazta. Eredmények: A megerősítő faktorelemzés szerint adataink nem illeszkedtek az eredeti, hatfaktoros, illetve a további kutatásokban javasolt kétfaktoros modellekre. A feltáró faktorelemzéssel négy faktort azonosítottunk (sajátértékek 1,43–6,68), amelyek együttesen a variancia 45,61%-át magyarázták (affektusok, elismerés, hasonló gondolkodás, korlátozás/autoritás). Az életminőség összefüggést mutatott az anyai és apai elismeréssel, a hasonló gondolkodással és a korlátozás/autoritással (rendre, F[4,175] = 14,01, p < 0,001, R2 = 0,24, F[4,166] = 17,14, p < 0,001, R2 = 0,29), míg az externalizáló tünetek az anyai elismeréssel, az anyai és apai korlátozással/autoritással és apához hasonló gondolkodással (rendre, F[4,137] = 6,10, p < 0,001, R2 = 0,15, F(4,133) = 8,69, p < 0,001, R2 = 0,21), az internalizáló tünetek az anyai és apai elismeréssel és az apai korlátozással voltak kapcsolatban (rendre, F[4,137] = 5,768, p < 0,001, R2 = 0,144, F[4,133] = 7,921, p < 0,001, R2 = 0,19). Diszkusszió: A Családi Kapcsolatok Teszt magyar változata vizsgálatunk alapján megbízható és érvényes mérőeszköz a családi kapcsolatok mérésére a gyermek perspektívájából.

PACAP (pituitary adenylate cyclase activating polypeptide) is a widespread neuropeptide with cytoprotective and anti-inflammatory effects. It plays a role in innate and adaptive immunity, but data are limited about gut-associated lymphoid tissue. We aimed to reveal differences in Peyer’s patches between wild-type (WT) and PACAP-deficient (KO) mice. Peyer’s patch morphology from young (3-months-old) and aging (12–15-months-old) mice was examined, along with flow cytometry to assess immune cell populations, expression of checkpoint molecules (PD-1, PD-L1, TIM-3, Gal-9) and functional markers (CD69, granzyme B, perforin) in CD3+, CD4+, and CD8+ T cells. We found slight differences between aging, but not in young, WT, and KO mice. In WT mice, aging reduced CD8+ T cell numbers frequency and altered checkpoint molecule expression (higher TIM-3, granzyme B; lower Gal-9, CD69). CD4+ T cell frequency was higher with similar checkpoint alterations, indicating a regulatory shift. In PACAP KO mice, aging did not change cell population frequencies but led to higher TIM-3, granzyme B and lower PD-1, PD-L1, Gal-9, and CD69 expression in CD4+ and CD8+ T cells, with reduced overall T cell activity. Thus, PACAP deficiency impacts immune dysfunction by altering checkpoint molecules and T cell functionality, particularly in CD8+ T cells, suggesting complex immune responses by PACAP, highlighting its role in intestinal homeostasis and potential implications for inflammatory bowel diseases.

Sex differences in pathogenesis and treatment of dyslipidemia in patients with type 2 diabetes and steatotic liver disease.
Previously published studies have shown that women with type 2 diabetes have a higher risk of atherosclerotic cardiovascular disease than men with type 2 diabetes. The exact reason for this is not yet known. The association between metabolic dysfunction-associated steatotic liver disease and type 2 diabetes appears to be bidirectional, meaning that the onset of one may increase the risk of the onset and progression of the other. Dyslipidemia is common in both diseases. Our aim was therefore to investigate whether there is a sex difference in the pathogenesis and management of dyslipidemia in patients with type 2 diabetes and steatotic liver disease with metabolic dysfunction. While the majority of published studies to date have found no difference between men and women in statin treatment, some studies have shown reduced effectiveness in women compared to men. Statin treatment is under-prescribed for both type 2 diabetics and patients with dysfunction-associated steatotic liver disease. No sex differences were found for ezetimibe treatment. However, to the best of our knowledge, no such study was found for fibrate treatment. Conflicting results on the efficacy of newer cholesterol-lowering PCSK9 inhibitors have been reported in women and men. Results from two real-world studies suggest that up-titration of statin dose improves the efficacy of PCSK9 inhibitors in women. Bempedoic acid treatment has been shown to be effective and safe in patients with type 2 diabetes and more effective in lipid lowering in women compared to men, based on phase 3 results published to date. Further research is needed to clarify whether the sex difference in dyslipidemia management shown in some studies plays a role in the risk of ASCVD in patients with type 2 diabetes and steatotic liver disease with metabolic dysfunction.

Simple SummaryPostoperative adjuvant chemotherapy generally improves survival in patients with breast cancer. However, adjuvant chemotherapy does not benefit all patients. There are considerations and guidelines that guide us as to whether or not chemotherapy is recommended to a particular patient. The decision is based on clinicopathologic features and may be aided by multigene assays. The Oncotype DX test is used worldwide. It makes the recommendation more accurate; however, there are possibilities to refine the process to make a more accurate decision. We investigated how we could move forward in recommending adjuvant chemotherapy.Background: The use of the Oncotype DX test reduces the rate of adjuvant chemotherapy recommendations. Few in-depth analyses have been performed on this decision-making process. Methods: We retrospectively analyzed patient data based on available Oncotype DX test results (RS) irrespective of nodal status at a single center. We collected recommendations from six oncologists, first without RS (pre-RS) and then with RS results (post-RS). We investigated changes in recommendations, agreement between oncologist decisions, and the effect of different National Comprehensive Cancer Network (NCCN) recommendation categories (for, against, and considering chemotherapy). Results: Data from 201 patients were included in the analysis. Recommendation of chemotherapy decreased by an average of 39.5%. Agreement improved substantially with RS, with a kappa value pre-RS of 0.37 (fair agreement) and post-RS of 0.75 (substantial agreement). Discordance remained substantial in cases where the NCCN recommendations considered chemotherapy only (32%). Pre-RS consensus against chemotherapy predicted low RS results (50 out of 51 patients). Post-RS consensus was highest in the NCCN chemotherapy recommendation group. Conclusions: The Oncotype DX test substantially improves decision accuracy in recommending adjuvant chemotherapy. It may be further improved with a consensus decision. In the case of pre-RS consensus against chemotherapy, the test can be spared.
