51歳男性．右上葉の11.5×8.8 cm大の肺癌疑いに対し，右上葉切除を施行した．組織学的には，非小細胞肺癌成分と肉腫様成分とが混在する低分化な腫瘍であり，癌肉腫や多形癌などの肉腫様癌，滑膜肉腫などが鑑別に挙がった．最終的には明確な特定の肉腫様成分への分化が指摘できなかった事，明確な神経内分泌的形質への分化を確定できなかった事，紡錘形細胞や巨細胞以外の細胞像も見られた事から，「肉腫あるいは肉腫様成分を含む低分化な非小細胞癌の総称」と定義される肉腫様癌の診断となった．pT4N2M0，stage IIIBであった．病理学的診断に苦慮した症例を経験したため報告する．

背景．近年，原発性非小細胞肺癌の周術期における免疫チェックポイント阻害薬（immune checkpoint inhibitor）の適応が広がっており，その1つに術前補助化学療法（neoadjuvant chemotherapy）としてニボルマブ＋プラチナ併用療法がある．症例．60歳，女性．原発性肺扁平上皮癌cT3N2M0 stage IIIB（遺伝子変異/転座陰性，programmed cell death ligand 1：80%）の診断となり術前化学免疫療法としてニボルマブ＋プラチナ併用療法を開始した．免疫関連有害事象（immune-related adverse events）のgrade 1の発熱が続き継続困難となり1コースのみの投与となったが，術前のCTで病変の縮小が得られ，その段階で右肺上葉切除＋気管支形成＋ND2a-2を施行した．切除検体の残存生存腫瘍は10%で著明な病理学的奏効を認めた．結論．術前化学免疫療法において免疫関連有害事象の発症に注意し，発症時は早期に診断・治療をした上で，腫瘍への治療効果を見極めながら状況に合わせて手術時期を逸しないことが重要と考える．

背景．非小細胞肺癌において，BRAF遺伝子変異は稀であり，初回治療にはダブラフェニブ＋トラメチニブ併用投与が推奨されている．症例．72歳女性．嗄声のために近医耳鼻咽喉科を受診し，肺癌を疑われたため当科に紹介となった．精査にて右上葉肺腺癌cT4N3M1a，cStage IVAと診断した．BRAF V600E変異を認めたため，ダブラフェニブ＋トラメチニブ併用投与にて治療を開始したところ，40か月以上の奏効が得られている．結論．BRAF V600E変異陽性肺癌に対してダブラフェニブ＋トラメチニブ併用投与が長期奏効した1例を経験したため，報告する．

Background. Hemophagocytic lymphohistiocytosis (HLH) is a fatal immune response syndrome characterized by abnormal hyperinflammatory and hyperferritinemia. Case. A patient in their 50s received combination ipilimumab plus nivolumab for recurrent pleural mesothelioma. Eight days after the start of immune checkpoint inhibitor treatment the patient was admitted to our hospital with a chief complaint of fever and gross hematuria. Although the patient was treated with antibiotics, their general condition began to worsen from the 11th day after admission. The patient was diagnosed with cytokine release syndrome and HLH and was treated with systemic corticosteroids and mycophenolate mofetil. Platelet transfusions for thrombocytopenia and continuous hemodiafiltration for acute renal failure were required. The patient gradually improved and was discharged from the hospital on the 39th day after admission. Conclusion. It should be noted that HLH is relatively frequent and can be serious in patients who receive combination therapy with ipilimumab plus nivolumab. An early diagnosis and administration of systemic corticosteroids and immunosuppressive agents is crucial when HLH is suspected during cancer immunotherapy.
